If a picture is worth a thousand words, then this tweet is more than enough to answer what an Adventure Plate is. Well done to @bringbackfsals for nailing the range of expressions possible through Portraits and Adventure Plates.
How To Make An Adventure Plate
Open the Character menu and select Adventure Plate

There are many options, and it may seem a little overwhelming at first. Not to worry; customizing your Adventure Plate is very easy, and the options that make the biggest impact are quick to change.
There are two main components to the Plate, which are the design and the content. You can let players know how you spend your time in Eorzea, i.e. Raiding, Levelling, Crafting, even Chatting and Roleplay…along with your weekday and weekend hours.
You could spend hours customizing every detail and flourish if you like, but I spend about 5 minutes and was pretty happy with mine. It has a medieval antique sort of aesthetic, which just feels very Final Fantasy (Age of Zodiac esque) to me. It’s total roleplay, imaginary-fantasy type fun, and it won’t be for everybody – that being said, when the feature was released in April 2020, it was clear that the Twitter playerbase took to it with enthusiasm and joy…with Plates and Portraits ranging from Criminal-Looking headshots, to … well… strip club-style ID cards (are they a thing?).
How To Change The Portrait On Your Adventure Plate
The first thing you need to do is apply your glamour and/or change your armor – this is what you’ll be wearing in the portrait. As you can see in the screenshot below, I have the Edit Portrait window open alongside the Adventure Plate.

You can interact directly with the camera in the Edit Portrait window; hold down the left mouse button to rotate and angle your model, use the scroll wheel to zoom in and out etc.. It will feel very intuitive as a player of Final Fantasy XIV.
The Portraits window lists all of your customized portraits; to apply a portrait to your Adventure Plate is as simple as Right Click (or bring up sub-menu with controller) and choose the 3rd option down (“Apply to Adventure Plate”). As you can see below…

After Apply to Adventure Plate, you will see the image update in real-time:
